Thank you for your interest in our evaluation services. The best way to start the process of obtaining an appointment is to complete an Intake so we can learn more about your needs for testing. Each intake is individually reviewed so please allow 7 days for an initial response. If you are a parent completing the Intake on behalf of your child, please complete the Intake (Child/Adolescent). The Adult Intake can be self-completed or completed by a collateral informant such as a parent or spouse. The referral forms are for professional use, for physicians, clinicians, or school-personnel to refer a patient, client, or student.
